As a Hospice Registered Nurse Case Manager, you will take full nursing responsibility for implementing and managing the Plan of Care for hospice patients. Your role involves continuous evaluation of both personal and professional performance, ensuring you deliver quality patient care. Collaborating with other healthcare professionals, you will develop tailored care strategies based on nursing theory, supporting the physical, psychosocial, emotional, and spiritual well-being of patients and families facing end-of-life challenges.

Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Assess and identify the physical, psychosocial, and environmental needs of patients and families, with reassessments every 15 days.
• Initiate and coordinate comprehensive plans of care.
• Document care plans, track goals, interventions, and patient/family responses meticulously.
• Work collaboratively with patients, families, attending physicians, and other interdisciplinary team members to provide daily care.
• Educate and supervise patients/families in self-care techniques as appropriate.
• Maintain accurate clinical notes regarding patient health and progress.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• Minimum of 1 year of clinical nursing experience.
• Current CPR certification.
• Valid driver's license, vehicle insurance, and access to a reliable vehicle, or public transport.
• Ability to work effectively in diverse home environments without regard to age, race, creed, color, sex, disability, or financial status.

Preferred Qualifications:
• Ability to work independently and take initiative.
• Strong communication, writing, and organizational skills.

The salary for this position ranges from $58,800 to $105,000 annually based on full-time employment, influenced by factors such as location, experience, and education.
