As the RN Case Manager, you will take full responsibility for delivering the Plan of Care for hospice patients, ensuring continuous evaluation and improvement in productivity and quality of care.

Your key responsibilities will include:
• Performing initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identifying and reassessing the physical, psychosocial, and environmental needs of patients and their families every 15 days.
• Coordinating and initiating the care plan while maintaining precise clinical notes on the patient’s condition.
• Documenting issues, goals, interventions, and responses related to hospice care.
• Collaborating with patients, families, attending physicians, and the interdisciplinary team to provide comprehensive care.

Experience Requirements:
One year of clinical experience is required.

License Requirements:
Current RN licensure in the state of practice, one year of clinical experience, current CPR certification, and a valid driver's license with dependable vehicle access are required.
