As a Hospice Registered Nurse Case Manager, you will take on a vital role in delivering comprehensive care to hospice patients. You will be responsible for developing and implementing the Plan of Care, collaborating with other healthcare professionals, and ensuring the physical, psychosocial, emotional, and spiritual well-being of patients and their families during this critical time.

Primary Responsibilities:
• Conduct initial nursing evaluations to determine patient eligibility for hospice services within 48 hours of referral.
• Identify and assess the physical, psychosocial, and environmental needs of patients and their families, with regular reassessments at least every 15 days.
• Initiate and coordinate individualized Plans of Care.
• Document patient progress, interventions, and family responses effectively.
• Collaborate daily with the patient/family, attending physicians, and other interdisciplinary team members to provide holistic care.
• Facilitate self-care techniques for patients and families when appropriate.
• Maintain accurate clinical notes regarding patient conditions.

Required Qualifications:
• Current and unrestricted RN licensure.
• At least 1 year of clinical experience.
• Current CPR Certification.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• Ability to provide care in diverse home settings.

State Specific Requirements:
• If located in Louisiana: Must possess 2+ years of full-time RN experience, with consideration for LPN experience in hospice.

Preferred Qualifications:
• Able to function autonomously with strong communication skills.
• Excellent writing and organizational abilities.

Compensation is competitive, ranging from $58,800 to $105,000 annually based on full-time employment, alongside a comprehensive benefits package, including 401k contributions, equity stock purchase options, and incentives subject to eligibility requirements.
