Your Role as a Hospice RN / RN Case Manager: • Serve as the primary point of contact for patients and families, providing guidance throughout their hospice journey. • Deliver and document skilled nursing care tailored to each patient's individualized Plan of Care, in their homes, assisted living facilities, or any location they call home. • Conduct initial and ongoing assessments to monitor patient conditions and adjust care as necessary. • Collaborate with an interdisciplinary team including physicians, LPNs, CNAs, social workers, and chaplains to develop personalized care plans. • Offer emotional, physical, and spiritual support to patients and their loved ones, providing education, comfort, and bereavement guidance. • Educate caregivers on providing safe and confident care for their loved ones with practical training and emotional reassurance. • Communicate critical updates regarding patient conditions to the care team, ensuring timely interventions. • Maintain accurate clinical documentation and adhere to infection control and compliance standards. • Support the organization's initiatives to improve quality of care and scheduling, ensuring compassionate and efficient services. • Help patients and families navigate essential topics including: - Medication administration - Hospice philosophy and available services - Symptom and pain management - End-of-life processes and expectations About You: What You'll Bring: • Active RN license in the state of employment (or eligibility to obtain). • At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related fields. • A genuine compassion for hospice care: empathetic, patient-centered, and dedicated. • Comfortable providing care in various settings, including private homes and care facilities. • Strong communication and critical thinking skills, especially in emotionally charged situations. • Valid driver’s license, auto insurance, and reliable transportation. • CPR certification required. Preferred Experience (Not Required): • Experience as a Hospital RN (Med-Surg, ICU, ER, PACU, telemetry, cardiac). • Experience as an Admissions RN, in wound care, float pool, or travel nursing. • Experience working with terminally ill patients or supporting serious illness.
