
Hospice Registered Nurse Case Manager in Artesia, MS
Job Description
As a Hospice Registered Nurse Case Manager, you will take on full responsibility for implementing the Plan of Care for each hospice patient. You will continuously evaluate and enhance both personal and professional performance to improve productivity and quality of care. Collaborating with multidisciplinary teams, you will develop and execute treatment strategies rooted in scientific nursing theory, aimed at supporting the physical, psychosocial, emotional, and spiritual well-being of patients and their families during end-of-life care.
Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Assess the patient’s/family’s physical, psychosocial, and environmental needs, with re-assessments at least every 15 days.
• Initiate and coordinate the comprehensive Plan of Care.
• Document patient problems, set appropriate goals, and record interventions and responses to hospice care.
• Collaborate daily with the patient, family, attending physician, and other interdisciplinary team members to provide comprehensive care.
• Instruct and guide patients and families in self-care techniques when appropriate.
• Maintain accurate clinical notes regarding the patient's condition and progress.

Qualifications:
• Current and unrestricted RN licensure.
• Minimum of 1 year of clinical experience.
• CPR Certification is required.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Ability to provide quality care in diverse home environments regardless of the client’s background.
Preferred Qualities:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
We offer a competitive salary range of $58,800 to $105,000 annually, based on factors such as local labor markets, qualifications, and experience. Our benefits package includes comprehensive options to support your wellbeing and future.
