Job Description
As the Hospice RN Case Manager, you will take full nursing responsibility for the delivery of tailored care plans for each patient. You will continuously evaluate and enhance your professional performance to improve both productivity and the quality of care. Collaborating with multidisciplinary teams, you will develop patient-centered plans that focus on physical, psychosocial, emotional, and spiritual well-being while addressing the complex needs of patients and families facing end-of-life issues.
Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services within forty-eight (48) hours of referral.
• Assess and address patients' and families' physical, psychosocial, and environmental needs, re-evaluating these requirements at least every fifteen (15) days.
• Initiate and manage the comprehensive plan of care.
• Document clinical findings, goals, interventions, and responses to care accurately.
• Collaborate with patients, families, attending physicians, and the interdisciplinary team (IDG) to ensure holistic patient care every day.
• Educate and guide patients and families in self-care techniques when appropriate.
• Maintain detailed clinical notes that reflect the patient’s condition and progress.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• A minimum of 1 year of clinical experience.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and dependable transportation or access to public transportation.
• Ability to work effectively in diverse home environments without regard to the client's age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
Pay is competitive and based on various factors, including local labor markets, education, experience, and certifications. The salary for this role ranges from $58,800 to $105,000 annually for full-time employment.
