
Hospice Registered Nurse Case Manager in Winchester, ID
Job Description
As a Hospice Registered Nurse Case Manager, you will have the vital responsibility of overseeing the delivery of care for each hospice patient, ensuring that their needs and preferences are met with compassion and professionalism. You will collaborate with a multi-disciplinary team to create and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families. An understanding of the challenges faced by those nearing the end of life is essential.
Key Responsibilities:
• Conduct thorough initial nursing evaluations to determine eligibility for hospice services within forty-eight (48) hours of referral.
• Assess the holistic needs of patients and their families, adjusting care plans as necessary, at least every fifteen (15) days.
• Develop and coordinate comprehensive plans of care.
• Maintain accurate and up-to-date documentation on patient conditions, interventions, and responses to care.
• Collaborate daily with patients, families, attending physicians, and other team members to provide cohesive care.
• Guide and support patients and families in self-care techniques when applicable.
• Keep detailed clinical notes that reflect patient progress and challenges.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• At least 1 year of clinical nursing experience.
• CPR Certification that is up to date.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
• Ability to provide care in diverse home settings, respecting the client’s background and situation.
Preferred Qualifications:
• Strong ability to work independently.
• Excellent communication, writing, and organizational skills.
Compensation for this role is competitive and is influenced by several factors, including local market conditions, educational qualifications, and work experience.
