Schedule: Monday-Friday 8am-5pm with holiday on-call rotation What We Offer: • Positive, team-oriented culture • Comprehensive benefits, including medical, dental, and vision, effective from the first of the month • 401(k) plan with a generous company match • Generous time-off accruals • Paid holidays • Mileage reimbursement • Tuition reimbursement opportunities • Employee referral program • Support for hospice certification with salary increase • Merit increases • Employee discount programs Key Responsibilities: • Take primary responsibility for a caseload of patients and families, covering all phases of the nursing process, including assessment, admission, planning, implementation, and evaluation. • Gather information regarding the physical, psychological, social, and spiritual needs of the patient, initiating interdisciplinary team interventions as necessary. • Communicate effectively with the attending physician, interdisciplinary team, and other agencies to coordinate optimal patient care. • Conduct regular patient visits to reassess needs and update care plans accordingly. • Perform clinical pronouncement of death and provide compassionate support to families. • Maintain accurate patient records, prepare timely care plans, and document care delivery by Home Health Aides according to guidelines. Qualifications: • Graduate from an accredited registered nursing program • Current registered nurse license in the state of practice • Current CPR and First Aid certification required • Knowledgeable in quality assessment and assurance procedures • Valid driver's license and proof of insurance required Apply today to make a difference in the lives of our patients and their families.
