Job Description
In the role of Hospice Registered Nurse Case Manager, you will take full responsibility for the quality of nursing care delivered to hospice patients. You will continuously assess your performance and adapt strategies to enhance productivity and care quality. Collaborating with other professionals, you'll develop individualized care plans that support the holistic well-being of both patients and their families.
Key Responsibilities:
• Conduct the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Assess and identify patients' and families' physical, psychosocial, and environmental needs, with regular re-assessments every 15 days.
• Initiate and manage the patient’s plan of care and ensure ongoing collaboration with the patient/family, attending physician, and interdisciplinary group.
• Maintain accurate and relevant documentation, including problems, goals, and patient/family responses to care.
• Guide and teach patients and families appropriate self-care techniques.
• Keep comprehensive clinical notes reflecting the patient's condition.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• At least 1 year of clinical experience.
• Current CPR Certification.
• Valid driver's license and vehicle insurance, with access to a reliable vehicle or public transportation.
• Ability to work in diverse home environments regardless of clients' backgrounds.
Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
The salary for this role will range from $58,800 to $105,000 annually for full-time employment, based on various factors like local labor markets and experience.
