Your Role as a Hospice RN / Case Manager:
• Act as the primary contact for patients and their families, guiding them through every aspect of their hospice journey.
• Provide skilled, hands-on nursing care as outlined in each patient's individualized Plan of Care in their home, assisted living, or wherever they live.
• Conduct initial and ongoing assessments to monitor patient conditions and adapt care as necessary.
• Work collaboratively with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to create and enhance personalized care plans.
• Offer emotional, physical, and spiritual support not only to patients but also to their loved ones, providing education, comfort, and bereavement guidance.
• Train caregivers in providing safe and effective care for their loved ones, combining practical skills with emotional support.
• Communicate critical updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
• Maintain precise and up-to-date clinical documentation while following compliance and infection control standards.
• Contribute to the organization’s quality improvement initiatives and support scheduling to ensure care is consistent, compassionate, and effective.
• Assist patients and families in understanding and navigating topics such as:
• Medication administration
• Hospice philosophy and services
• Symptom and pain management
• End-of-life processes and expectations
About You:
• Active RN license in the state of employment (or eligible to obtain).
• At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or other relevant nursing fields.
• A genuine compassion for hospice care: empathetic, caring, and patient-centered.
• Comfortable providing care in diverse settings including private homes and care facilities.
• Excellent communication and critical thinking skills, especially in sensitive situations.
• Valid driver’s license, auto insurance, and reliable transportation are required.
• CPR certification is mandatory.
Preferred Experience (Optional):
• Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
• Experience as an Admissions RN, in wound care, float pool, or travel nursing roles.
• Engagement with terminally ill patients or serious illness support is a plus.
