Hospice Registered Nurse Case Manager in Casstown, OH

Your Responsibilities:
• Lead the ongoing assessment and implementation of the patient's plan of care, collaborating with our spiritual care coordinator, social worker, hospice aide, and medical director
• Conduct thorough evaluations and develop comprehensive care plans tailored to address patient needs
• Regularly communicate changes in patient conditions and coordinate with healthcare providers to adapt care as required

Qualifications:
• Education: Associate Degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required; Bachelor of Science in Nursing (BSN) preferred
• Licensure: Must hold a current license to practice as a Registered Nurse (RN)
• Certification: Current CPR/FA certification required
• Transportation: Reliable transportation to and from care sites
• Experience: One (1) year of professional nursing experience in hospice or a similar care setting preferred
