
Hospice Registered Nurse Case Manager in Carson City, NV
Job Description
As the Registered Nurse Case Manager, you will take on the vital role of delivering personalized care to hospice patients. You will regularly evaluate your performance to enhance both productivity and the quality of care provided. Collaborate with other professionals to design and implement tailored care plans that address the physical, psychosocial, emotional, and spiritual needs of patients and their families, especially those facing end-of-life challenges.
Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Assess and identify the physical, psychosocial, and environmental needs of patients and families, with reassessments scheduled no less than every 15 days.
• Develop and oversee the implementation of individualized plans of care.
• Document relevant clinical information, including patient issues, goals, interventions, and responses to care.
• Partner with patients, families, physicians, and interdisciplinary team members to ensure holistic care.
• Educate and guide patients and families in self-care techniques whenever appropriate.
• Maintain accurate clinical notes related to patient health status and treatment.

Required Qualifications:
• Active and unrestricted RN licensure.
• Minimum of 1 year of clinical nursing experience.
• Current CPR Certification.
• Valid driver’s license, vehicle insurance, and reliable transportation.
• Ability to work effectively in diverse home environments without bias.
Preferred Qualifications:
• Capability to work independently and manage time effectively.
• Excellent communication, writing, and organizational skills.
Compensation for this position ranges from $58,800 to $105,000 annually for full-time employment, depending on various factors including location, experience, and education. Additional benefits include a comprehensive package, incentive programs, and 401k contributions, subject to eligibility requirements.
