
Hospice Registered Nurse Case Manager in Broussard, LA
Job Description
As a Hospice RN, you will:
• Develop trusted relationships with patients and families as a dedicated end-of-life care expert.
• Be recognized and valued by patients and their families for your essential role.

Your key responsibilities will include:
• Assuming full nursing responsibility for delivering personalized Plans of Care for each hospice patient.
• Conducting initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identifying and re-assessing the physical, psychosocial, and environmental needs of patients and families at least every 15 days.
• Initiating and coordinating the Plan of Care.
• Documenting problems, goals, interventions, and patient/family responses to care.
• Collaborating with patients, families, physicians, and the interdisciplinary group for comprehensive care.
• Instructing and supervising patients/families in self-care techniques as appropriate.
• Maintaining accurate clinical notes on patient conditions.
Experience Requirements:
• One year of clinical experience is required.
License Requirements:
• Current RN licensure in the state of practice and one year of clinical experience.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
Additional State Specific Requirements:
• In Louisiana, a minimum of two years of full-time experience as an RN is required, or two years of full-time clinical experience in hospice care as a licensed practical nurse may be substituted for the required RN experience.
