
Hospice Registered Nurse Case Manager in Oxford, AR
Job Description
As a Hospice Registered Nurse Case Manager, you'll take full responsibility for delivering personalized care to each hospice patient through a carefully tailored Plan of Care. You'll continuously assess and refine your performance to enhance productivity and the quality of care provided. Collaborate with other healthcare professionals to design and implement effective treatment strategies that foster physical, emotional, and spiritual well-being for patients and their families during critical times.
Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services, typically within forty-eight (48) hours of referral.
• Identify and address the physical, psychosocial, and environmental needs of patients and their families, re-evaluating every fifteen (15) days.
• Develop and coordinate comprehensive plans of care.
• Document care goals, interventions, and patient/family responses accurately.
• Work collaboratively with patients, families, attending physicians, and interdisciplinary teams to facilitate daily care.
• Educate and supervise patients and families in self-care techniques as needed.
• Maintain precise clinical notes regarding patient conditions.

Required Qualifications:
• Current and unrestricted RN licensure.
• Minimum of 1 year of clinical experience.
• Current CPR Certification.
• Valid driver’s license, vehicle insurance, and reliable transportation.
• Ability to provide care across diverse home situations.
Preferred Qualifications:
• Ability to work independently.
• Excellent communication, writing, and organizational skills.
Compensation is based on multiple factors, including local labor markets, education, and experience.
