
Hospice Registered Nurse Case Manager in Washington, NJ

Compensation: $87,750.00 per year
Sign-On Bonus: $8,000.00 paid over 1 year
We provide a robust benefits package that includes a medical stipend, dental and vision insurance, paid time off, a 401K plan, and tuition reimbursement.
Schedule: Monday to Friday, 8:30 AM to 4:30 PM, with occasional on-call responsibilities.
In your role, you will:
• Explain and coordinate patients' rights and advanced directives, while clarifying the Medicare Hospice Benefit as needed.
• Conduct ongoing physical and psychosocial assessments of hospice patients.
• Implement and manage an established plan of care, providing updates to the team leader or primary nurse.
• Collaborate with the interdisciplinary team to address new nursing diagnoses and adjust care plans accordingly.
• Supervise hospice aides and LPN staff to ensure exceptional care delivery.
• Assist patients and families in obtaining necessary durable medical equipment and supplies.
• Monitor and document symptoms and changes in patient conditions effectively.
• Communicate with physicians, families, and healthcare professionals to discuss patient status and needs.
• Educate patients and families, documenting the teaching process and tracking responses.
• Promote patient and family independence through self-direction initiatives.
• Administer medications in accordance with agency policies, ensuring accurate documentation of all actions.
• Participate in our at-home respite program, providing essential support to caregivers while delivering direct patient care.
• Complete all necessary chart documentation within 48 hours of care provided.
• Maintain a caseload that meets agency productivity standards.
• Engage with the Nursing Supervisor and healthcare team in planning and coordinating patient services.
• Participate in the on-call system for scheduled encounters.
• Implement appropriate infection control practices to safeguard patient health.
• Protect all patient information, disclosing it only to authorized individuals.
• Participate in mandatory educational programs and attend at least 9 out of 12 nursing meetings annually.
• Adhere to dress code and maintain a professional appearance while on duty.
• Perform additional duties as assigned to support our mission.
Required Skills/Abilities:
• Strong nursing assessment and documentation skills.
• Clinical knowledge to support end-of-life care practices.
• Resourcefulness and compassion in serving patients and their families during challenging times.
• Outstanding interpersonal and communication skills.
• Ability to comprehend spoken and written English and communicate clearly.
• Organizational skills to effectively manage patient needs.
• Proficiency in computer applications, including Microsoft Office and electronic medical records.
Education and Experience:
Required Education: Diploma, Associate, or Bachelor of Science in Nursing; BSN preferred. Must be a licensed Registered Nurse in New Jersey and possess licensure in Pennsylvania if practicing there.
Preferred Education: CPR certification is advantageous.
Required Experience: Minimum of one year in adult, pediatric, or medical/surgical clinical settings.
Preferred Experience: Prior experience in a home care or hospice setting is highly preferred.
