
Hospice Registered Nurse Case Manager in Bright, IN

Your Responsibilities:
• Manage a patient and family caseload, overseeing all phases of the nursing process, including assessment, eligibility, admission, planning, implementation, and evaluation.
• Gather and assess information regarding the patients' physical, psychological, social, and spiritual needs, and initiate interventions with the interdisciplinary team.
• Communicate effectively with the attending physician and other team members to facilitate optimal care.
• Conduct regular visits to reassess patient needs and update care plans accordingly.
• Perform clinical pronouncements of death and provide compassionate support to families during difficult times.
• Maintain accurate patient records, progress notes, and timely care plans.
• Supervise the Home Health Aide's care delivery and document compliance with all guidelines.
Qualifications:
• Graduate from an accredited registered nursing program.
• Current licensed registered nurse in the state of practice.
• CPR and First Aid certification required.
• Knowledge of quality assessment and assurance processes.
• Valid driver's license and proof of insurance are mandatory.
