Hospice Registered Nurse Case Manager Salary: $80-89k/yr BOESchedule: Full time | M-F| 8:00AM-5:00PM +(on-call rotation)Location: Dayton Ohio and surrounding areas Requirements of the RNCM role:Licensed Registered NurseBLS Certified Valid Drivers License and Auto insurance CoverageReliable transportation A minimum of 1 year of Clinical experienceHospice experience preferred About the RNCM role:Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. Perform evaluations for hospice care along with informational visits as needed.Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care. Submit complete, accurate, and relevant clinical notes promptly.Participate in IDG and case conferencesPerforms on-call duties per policy and needConducts an initial and ongoing comprehensive assessment of the patientsDevelop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care. Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed.Provide care plan to Hospice Aide and LPN as part of the IDG team.
