
Hospice Registered Nurse Case Manager in Greenleaf, ID
Job Description
As a Hospice Registered Nurse Case Manager, you will play a crucial role in delivering compassionate and high-quality care to hospice patients. You will create and oversee the patient's Plan of Care, collaborating with a skilled team of healthcare professionals to promote physical, emotional, psychosocial, and spiritual well-being. This role requires sensitivity to the needs of patients and families confronting the end of life, ensuring dignified and respectful care.
Key Responsibilities:
• Conduct initial nursing evaluations to determine hospice service eligibility within 48 hours of referral.
• Assess and identify the physical, psychosocial, and environmental needs of the patient and family, with evaluations occurring no less than every 15 days.
• Initiate and coordinate comprehensive Plans of Care tailored to each patient's requirements.
• Maintain thorough documentation of patient problems, goals, interventions, and responses to care.
• Collaborate closely with patients, families, attending physicians, and interdisciplinary team members to ensure holistic care.
• Provide instruction and supervision for patient and family self-care whenever appropriate.
• Keep accurate and comprehensive clinical notes concerning the patient's condition and progress.

Required Qualifications:
• Current and unrestricted RN licensure.
• Minimum of 1 year clinical nursing experience.
• Current CPR Certification.
• Valid driver’s license and access to a reliable vehicle or public transportation.
• Capability to function effectively in varied home care situations, respecting the diverse backgrounds of clients.

• Strong communication, writing, and organizational skills.
Your compensation will be competitive, influenced by factors such as local labor markets, education, and experience. In addition to your salary, we offer a comprehensive benefits package, incentive programs, stock purchase options, and 401k contributions. The salary range for this role is between $58,800 and $105,000 annually for full-time employment. We comply with all minimum wage laws applicable.
