
Hospice Registered Nurse Case Manager in Caldwell, ID
Job Description

• Conduct the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify and assess the physical, psychosocial, and environmental needs of patients and families, re-evaluating no less than every 15 days.
• Initiate and coordinate the Plan of Care while ensuring accurate clinical documentation.
• Document problems, goals, interventions, and patient/family responses to care.
• Collaborate daily with patients, families, attending physicians, and interdisciplinary team members to provide comprehensive care.
Experience Requirements: Minimum one year of clinical experience.
License Requirements:
• Current RN licensure in the state of practice.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
