
Hospice Registered Nurse Case Manager in Ellisville, MS
Job Description
As a Hospice Registered Nurse Case Manager, you will take full nursing responsibility for implementing and overseeing each patient's Plan of Care. You will continuously assess and enhance your performance to ensure optimal productivity and outstanding quality of care. Collaborating with a multidisciplinary team, you will create tailored care plans and implement evidence-based treatment strategies that address the physical, psychosocial, emotional, and spiritual needs of patients and families facing end-of-life challenges.
Key Responsibilities:
• Conduct initial nursing evaluations within 48 hours of referral to determine hospice eligibility.
• Identify the patient's and family's needs and perform re-assessments at least every 15 days.
• Initiate and coordinate the plan of care.
• Document issues, set goals, intervene appropriately, and record patient/family responses.
• Work closely with patients, families, attending physicians, and interdisciplinary team members to provide comprehensive care.
• Teach and support patients and families in self-care techniques as appropriate.
• Maintain thorough and accurate clinical notes on the patient's condition.

Required Qualifications:
• Active RN licensure in your state.
• Minimum of 1 year of clinical experience.
• Current CPR certification.
• Valid driver's license and insurance, with access to a reliable vehicle or public transportation.
• Ability to work sensitively with clients regardless of their background or circumstances.
State Specific Requirements:
• For Mississippi, 1 year of RN experience or 3 years as an LPN may be substituted for RN experience.
Preferred Qualifications:
• Strong ability to work independently.
• Excellent communication, writing, and organizational skills.
We offer a competitive salary based on various factors, including local labor markets and individual qualifications. The salary for this role ranges from $58,800 to $105,000 annually for full-time employment. Our benefits package includes comprehensive health options, equity stock purchase, and a 401k plan.
