Hospice Registered Nurse Case Manager in Cincinnati, OH

Hospice Registered Nurse Case Manager Salary: $90-93k/yr BOE Schedule: Full time | M-F| 8:00AM-5:00PM +(on-call rotation) Location: Cincinnati, Ohio, and surrounding areas Requirements of the RNCM role: • Licensed Registered Nurse • BLS Certified • Valid driver's License and Auto insurance Coverage • Reliable transportation • A minimum of 1 year of Clinical experience • Hospice experience preferred About the RNCM role: • Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met. • Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis. • Perform evaluations for hospice care along with informational visits as needed. • Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care. • Submit complete, accurate, and relevant clinical notes promptly. • Participate in IDG and case conferences • Performs on-call duties per policy and need • Conducts an initial and ongoing comprehensive assessment of the patients • Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care. • Supervise and evaluate the care given by the hospice Aide at a minimum of once every 14 days or as needed. • Provide care plan to Hospice Aide and LPN as part of the IDG team. • Perform death visits per company policy • Participate in patient's discharge planning process Why work with Care 360 Hospice: • Medical, dental, and vision health Insurance • Paid Time Off • 401(k) Matching • Life Insurance • Growth opportunities NEXT STEPS We appreciate your interest in our Hospice Registered Nurse Case Manager.
