What You'll Do:
• Take primary responsibility for a caseload of patients and families, overseeing the nursing process from assessment to evaluation
• Gather comprehensive data regarding patients’ physical, psychological, social, and spiritual needs to inform care
• Foster communication among the attending physician, interdisciplinary team, and other organizations to ensure optimal patient care
• Conduct regular visits to reassess patient needs and adjust care plans accordingly
• Perform clinical pronouncement of death and provide compassionate support to grieving families
• Maintain accurate and timely patient records, charts, and care plans
• Supervise and document the care provided by Home Health Aides in compliance with Federal, State, and company guidelines
Qualifications:
• Graduate of an accredited registered nursing program
• Registered Nurse license in your state of practice
• Current CPR and First Aid Certification
• Strong knowledge of quality assessment and assurance procedures
• Valid driver's license and proof of insurance are required
