Job Description
As a Hospice Registered Nurse Case Manager, you will take full nursing responsibility for the delivery of the Plan of Care for each hospice patient. You will continuously assess your own performance and implement improvements to enhance productivity and the quality of care provided. Collaborate with a multidisciplinary team to craft and execute the Plan of Care that supports the physical, psychosocial, emotional, and spiritual well-being of patients and their families during this sensitive time.
Key Responsibilities:
• Conduct initial nursing evaluations to determine hospice eligibility within 48 hours of referral.
• Identify and assess the physical, psychosocial, and environmental needs of patients and families, re-evaluating at least every 15 days.
• Develop, initiate, and coordinate the personalized Plan of Care.
• Document identified problems, set goals, implement interventions, and track patient/family responses to care.
• Work collaboratively with patients, families, attending physicians, and team members to provide daily care.
• Instruct and coach patients and families on self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding each patient's condition.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice.
• Minimum of 1 year of clinical experience.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and reliable access to a vehicle or public transportation.
• Able to work in diverse home environments, respecting all backgrounds, including age, race, creed, color, sex, disability, or financial condition.
Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
Salary based on factors including local labor markets, education, work experience, and certifications, with annual compensation ranging from $58,800 to $105,000 for full-time positions.
