Job Description
In this vital role, you will oversee the comprehensive nursing care for hospice patients, creating and maintaining personalized Plans of Care and collaborating with a team of healthcare professionals to ensure the highest quality of care. You will bring comfort and dignity to patients and their families during a challenging time, focusing on physical, emotional, and spiritual well-being.
Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral.
• Identify and reassess the physical, psychosocial, and environmental needs of patients and families at least every 15 days.
• Develop and coordinate the Plan of Care in collaboration with the patient/family, attending physician, and interdisciplinary team (IDG).
• Document patient goals, interventions, and responses to care effectively.
• Guide and educate patients and families in self-care practices when appropriate.
• Keep accurate clinical notes reflecting the patient's condition and care provided.

Required Qualifications:
• Current, unrestricted RN licensure in the state of practice.
• At least 1 year of clinical nursing experience.
• Current CPR Certification.
• Valid driver's license with vehicle insurance and reliable transportation, or access to public transit.
• Ability to provide care in diverse home settings, respecting all individuals regardless of background.
State Specific Requirements:
• For Mississippi: Must have 1+ years of full-time experience as an RN, or three years as an LPN in a healthcare setting may be substituted.
Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
Compensation ranges from $58,800 to $105,000 annually based on factors such as experience, role, and local labor markets.
