Primary Responsibilities:
• Coordinate comprehensive care, continuously assess patient and family needs, and implement the interdisciplinary care plan.
• Deliver services in alignment with physician orders and the established plan of care (POC).
• Ensure compliance with federal and state regulations in all aspects of care delivery.

Qualifications:
• Graduation from an approved clinical education program.
• Current licensure to practice as an RN.
• At least one year of experience as a licensed professional.
• Basic technology skills, including mobile device operation.

Preferred Experience:
• Experience in Medicare home health or hospice settings is preferred.

Additional requirements include:
• Valid state driver’s license.
• Automobile liability insurance as required by law.
• A dependable vehicle in good working condition.
• The ability to drive safely in various weather conditions.
• CPR certification for healthcare providers.
