What You’ll Do
• Provide hospice nursing visits in patient homes and facility settings
• Deliver pain and symptom management, clinical assessments, and patient education
• Support patients and families through end-of-life transitions with confidence and compassion
• Collaborate closely with physicians, social workers, chaplains, and care team members
• Communicate clearly with families, facility staff, and referral partners
• Complete accurate, timely documentation in the EMR
Who We’re Looking For
• Active Minnesota RN license
• Experience in hospice, home care, palliative care, oncology, or end-of-life nursing
• Comfortable working independently in the field
• Strong communication skills and emotional intelligence
• Reliable transportation and valid driver’s license
• A nurse who wants to help build something meaningful
