Key Responsibilities as a Hospice RN / RN Case Manager:
• Serve as the primary contact for patients and their families, guiding them through each step of their hospice journey.
• Provide and document tailored nursing care based on each patient's individualized Plan of Care, whether it’s in their home, assisted living, or any setting they choose.
• Conduct initial and ongoing assessments to monitor patient conditions and adapt care as needed.
• Work collaboratively with an interdisciplinary team—including physicians, LPNs, CNAs, social workers, chaplains, and others—to create and refine personalized care plans.
• Offer emotional, physical, and spiritual support to patients and their loved ones, providing education, comfort, and bereavement guidance.
• Educate caregivers on safe and confident care practices, offering practical training and emotional reassurance.
• Communicate essential updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
• Maintain accurate and up-to-date clinical documentation while adhering to infection control and compliance standards.
• Support the organization’s quality improvement and scheduling initiatives to ensure compassionate and efficient care.
• Help patients and families navigate topics such as medication administration, hospice philosophy, symptom and pain management, and end-of-life processes.
Qualifications - What You'll Bring:
• Active RN license in the state of employment (or eligibility to obtain).
• At least 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or similar nursing fields.
• A deep-seated compassion for hospice care: empathetic and patient-centered.
• Experience providing care in diverse settings, including private homes and facilities.
• Excellent communication and critical thinking skills, especially in emotionally sensitive situations.
• Valid driver's license, auto insurance, and reliable transportation.
• Current CPR certification is required.
Preferred Experience (Not Required):
• Hospital RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).
• Experience as an Admissions RN, in wound care, float pool, or travel nursing.
• Exposure to terminally ill patients or serious illness support.
