As a Registered Nurse Case Manager, you will take full responsibility for the delivery of the Plan of Care for hospice patients. You will continuously assess both personal and professional performance, implementing changes as necessary to elevate productivity and quality of care. Collaborating with a team of professionals, you will develop and execute individualized care plans that address the physical, psychosocial, emotional, and spiritual needs of each patient and their family, especially during the difficult time of facing death and dying. • This is a Full Time position focused on Home Visits for Hospice Care. • RN license in NC is required. • Business Hours: Monday to Friday, 8 AM - 4:30 PM. • Agency covers a 50-mile radius, and candidates must be able to see patients within this area. Key Responsibilities: • Conduct initial nursing evaluations to determine eligibility for hospice services within 48 hours of referral. • Assess and reassess the patient's/family's physical, psychosocial, and environmental needs no less than every 15 days. • Initiate and coordinate comprehensive plans of care. • Document problems, goals, interventions, and the patient's/family's response to care. • Work collaboratively with the patient/family, attending physician, and interdisciplinary team members for optimal daily care. • Educate and supervise patients/families in self-care techniques when appropriate. • Maintain detailed and accurate clinical notes regarding the patient's condition. Required Qualifications: • Current and unrestricted RN licensure in North Carolina. • Minimum of 1 year clinical experience. • Current CPR Certification. • Valid driver's license, vehicle insurance, and reliable vehicle or access to public transportation. • Ability to work in diverse home situations regardless of age, race, creed, color, sex, disability, or financial status of clients. Preferred Qualifications: • Able to work independently. • Strong communication, writing, and organizational skills. Compensation is competitive and based on various factors including local labor markets, education, and experience.
