What You?ll Do
• Provide Holistic Care: Deliver skilled nursing care, including pain management, symptom control, and emotional support, tailored to each patient?s needs.
• Collaborate with Heart: Work closely with interdisciplinary teams, including physicians, social workers, and chaplains, to create personalized care plans.
• Empower Families: Educate and support patients? loved ones, offering guidance and compassion during challenging times.
• Document with Care: Maintain accurate, timely records to ensure seamless care coordination.
• Be a Beacon of Comfort: Offer presence, empathy, and advocacy for patients in their home or facility settings.
What You Bring
• Licensure: Active RN license in Wisconsin (BSN preferred but not required).
• Experience: Minimum 1-2 years of nursing experience; hospice, palliative care, or home health experience is a plus.
• Compassion: A heart for end-of-life care, with strong emotional resilience and empathy.
• Skills: Excellent assessment, communication, and time-management skills.
• Flexibility: Ability to travel locally to patient homes/facilities and work on-call shifts as needed.
