Your Role as a Hospice RN/RN Case Manager: • You will be the primary point of contact for patients and families, providing guidance throughout their hospice journey. • Deliver and document comprehensive nursing care based on each patient's individualized Plan of Care in their homes, assisted living facilities, or wherever they reside. • Conduct initial and ongoing assessments to monitor patient conditions and adjust care protocols as necessary. • Collaborate with an interdisciplinary team—comprising physicians, LPNs, CNAs, social workers, chaplains, and others—to develop refined, personalized care plans. • Offer emotional, physical, and spiritual support not only to the patients but also to their caregivers, providing education, comfort, and bereavement resources. • Instruct caregivers on how to safely and confidently care for their loved ones, combining practical training with emotional support. • Communicate important updates regarding patient conditions to the care team, ensuring timely interventions are made. • Maintain accurate and up-to-date clinical documentation while adhering to infection control and compliance standards. • Support the organization’s quality improvement initiatives and scheduling efforts to ensure that care is compassionate, consistent, and efficient. • Assist patients and families in navigating critical topics such as: - Medication administration - Hospice philosophy and services - Symptom and pain management - End-of-life processes and expectations Qualifications - What You Will Bring: • Active RN license in the state of employment (or eligibility to obtain one). • 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields. • A genuine passion for hospice care: compassionate, empathetic, and patient-centered. • Comfortable providing care in diverse settings, including patients’ homes and various healthcare facilities. • Strong communication and critical thinking skills, especially in emotionally sensitive situations. • Valid driver's license, automobile insurance, and reliable transportation. • CPR certification required.
