
Hospice Registered Nurse - On-Demand in Benton, MO
Job Description
As a Registered Nurse Case Manager, you will take on full nursing responsibility for the delivery of individualized care to each hospice patient. You will be key in evaluating and improving both personal and professional performance to boost productivity and enhance the quality of care provided. You will work collaboratively with other professionals to develop comprehensive Plans of Care that support the holistic well-being of each patient, addressing physical, psychosocial, emotional, and spiritual needs.
Primary Responsibilities:
• Conduct initial nursing evaluations within forty-eight (48) hours of referral to determine hospice service eligibility.
• Assess and address the physical, psychosocial, and environmental needs of patients and families, with regular re-evaluations every fifteen (15) days.
• Initiate and coordinate the patient’s Plan of Care.
• Document critical issues, appropriate goals, interventions, and responses to care accurately.
• Collaborate closely with patients, families, physicians, and interdisciplinary team members in daily care provisions.
• Provide education and supervision for patient/family self-care techniques as needed.
• Maintain thorough and relevant clinical notes regarding the patient’s ongoing condition.

Required Qualifications:
• Valid and unrestricted RN license in the state of practice.
• Minimum of 1 year of clinical experience.
• Current CPR Certification.
• Valid driver's license, dependable vehicle, and valid vehicle insurance, or access to public transportation.
• Ability to provide care in various home settings, regardless of age, race, creed, color, sex, disability, or financial condition of clients.
Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.
We offer competitive pay based on experience along with an extensive benefits package, including equity stock purchase and 401k contributions (subject to eligibility). The annual salary for this role typically ranges between $58,800 and $105,000 for full-time employment.
