Type of Opportunity: Full time FTE: 1 Job Exempt: No Work Shift: Days (United States of America) Responsibilities: • Conduct timely and comprehensive assessments of patients' physical, mental, and spiritual needs. • Deliver skilled nursing care according to physician plans while maintaining documentation standards. • Collaborate with the multidisciplinary team and coordinate patient care effectively. • Continuously evaluate the effectiveness of interventions and adjust care plans as necessary. • Manage caseload by maintaining productivity standards and ensuring timely communication with patients regarding schedule changes. • Precept and mentor new nurses and students, serving as a professional role model. • Engage in quality improvement initiatives as part of the agency's commitment to excellence. • Provide holiday and weekend coverage as required, participating in a rotating schedule. • Perform other duties as needed to support the team's operational success. Qualifications: • Associate Degree in Nursing. • Valid Registered Nurse license in New Mexico. • Minimum of one year of experience in direct patient care or home health care. • Current driver's license and auto insurance.
