Summary:
As a Hospice Registered Nurse in an outpatient setting, you will play a vital role in ensuring compassionate end-of-life care for patients and their families. Your responsibilities will include direct patient care and case management, focusing on achieving clinical outcomes while maintaining quality and utilization standards. You will collaborate with a multidisciplinary team to develop and implement comprehensive care plans tailored to individual patient needs.

Responsibilities:
• Conduct timely and comprehensive assessments of patients' physical, mental, and spiritual needs.
• Deliver skilled nursing care according to physician plans while maintaining documentation standards.
• Collaborate with the multidisciplinary team and coordinate patient care effectively.
• Continuously evaluate the effectiveness of interventions and adjust care plans as necessary.
• Manage caseload by maintaining productivity standards and ensuring timely communication with patients regarding schedule changes.
• Precept and mentor new nurses and students, serving as a professional role model.
• Engage in quality improvement initiatives as part of the agency's commitment to excellence.
• Provide holiday and weekend coverage as required, participating in a rotating schedule.
• Perform other duties as needed to support the team's operational success.
Qualifications:
• Associate Degree in Nursing.
• Valid Registered Nurse license in New Mexico.
• Minimum of one year of experience in direct patient care or home health care.
• Current driver's license and auto insurance.
