As a Hospice Registered Nurse, you will take on the vital role of Case Manager, overseeing the comprehensive delivery of care for our hospice patients. This position requires a commitment to continuous self-improvement and responsiveness to the needs of your patients, ensuring the highest quality of care. You will collaborate with a multidisciplinary team to develop and implement a Plan of Care that supports the overall well-being—physical, emotional, psychosocial, and spiritual—of the patients and their families, particularly during their most challenging times.

Key Responsibilities:
• Conduct initial nursing evaluations to assess eligibility for hospice services within 48 hours of referral.
• Identify and address the physical, psychosocial, and environmental needs of patients and their families, with regular reassessments at least every 15 days.
• Initiate and coordinate personalized care plans.
• Maintain accurate documentation regarding patient issues, goals, interventions, and responses to care.
• Work collaboratively with patients, families, physicians, and other team members to provide ongoing support and care.
• Educate and assist patients and families in self-care techniques when appropriate.
• Record detailed and relevant clinical notes about the patient's condition.

Required Qualifications:
• Current and unrestricted RN licensure.
• Minimum of 1 year of clinical experience.
• Valid CPR Certification.
• Current driver's license, valid vehicle insurance, and access to reliable transportation.
• Capability to work in diverse home environments regardless of the client's background.

State Specific Requirements:
• For Louisiana applicants: 2+ years of full-time RN experience or 2 years as a licensed practical nurse in hospice care may substitute for RN experience.

Preferred Qualifications:
• Ability to work independently.
• Strong communication, writing, and organizational skills.

Compensation is competitive and varies based on multiple factors, including local market rates, experience, and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package, recognition programs, equity stock purchase options, and 401k contributions—subject to eligibility requirements. The annual salary for this role can range from $58,800 to $105,000 for full-time employment. We adhere to all applicable minimum wage laws.
