Overview:
Position: RN/Registered Nurse, Hospice
Position Type: Full-Time
Remote/Virtual Position: No
Coverage Area: Hamden, New Haven and Orange
Salary: $90,000 - $105,000/year
Schedule: Monday - Friday 8am - 5pm plus 1 weekend day back up on call every 6 weeks
$10,000 Sign-On Bonus

Be the Best Hospice RN You Can Be
If you meet the following qualifications, we want to hear from you:
• Associates degree preferred
• Registered Nurse license required in state(s) practicing
• Current and valid CPR certification preferred
• Minimum one year of relevant experience preferred
• Experience in hospice, home health, or other nursing specialties (geriatrics, pediatrics, etc.)
Key Responsibilities:
• Conduct comprehensive nursing assessments and develop individualized care plans with the interdisciplinary team (IDG), addressing the needs of patients.
• Maintain accurate documentation of care, interventions, and patient status, ensuring compliance with all regulations.
• Coordinate care activities, including scheduling visits, managing medications, and facilitating communication between healthcare providers.
• Provide education and support to patients and caregivers, while also responding to urgent needs.
• Supervise LPNs, LVNs, and hospice aides as needed, and engage in IDG meetings and patient conferences.
• Evaluate patient status through various means and identify needs for additional care.
