Your Impact as a Hospice RN: • Conduct thorough nursing assessments and craft personalized care plans with the interdisciplinary team (IDG), addressing the diverse needs of patients—physical, emotional, social, and spiritual. • Maintain accurate and timely documentation of care, ensuring compliance with protocols and regulatory standards. • Coordinate essential care activities, including scheduling visits, managing medications, and fostering effective communication among healthcare providers. • Educate and support patients and families, promptly addressing urgent needs during on-call or admission periods. • Supervise LPNs, LVNs, and hospice aides, actively participating in IDG meetings and ongoing clinical education opportunities. • Assess patient status through various methods (phone, telehealth, or in-person) to identify needs for additional care or pre-hospice services.
