Position: Hospice Registered Nurse (RN)
Position Type: Full-Time
Remote/Virtual Position: No
Coverage Area: Visiting patients in Davidson County (Nashville/Hermitage) and Cheatham County.

Salary: $80,000-90,000 plus mileage

Be the Best Hospice RN You Can Be
If you possess the following qualifications, we would love to connect with you:
• Associate's degree preferred
• Valid Registered Nurse license in the practicing state
• Current CPR certification preferred (required in Georgia)
• Minimum of one year of experience preferred (required in California)
• Experience in hospice, home health, or relevant patient care areas (geriatrics, pediatrics, etc.)
Responsibilities:
• Conduct comprehensive nursing assessments and develop individualized care plans with the interdisciplinary team (IDG) to meet physical, emotional, social, and spiritual needs.
• Ensure timely and accurate documentation of care and patient status, adhering to established protocols and regulations.
• Coordinate care activities including scheduling visits, managing medications, and facilitating communication among healthcare providers to support quality care.
• Educate and support patients and caregivers, addressing urgent needs during on-call or admissions assignments.
• Supervise LPNs, LVNs, and hospice aides as necessary, engaging in IDG meetings, patient conferences, and continuous clinical education.
• Support pre-hospice services by assessing patient status via phone, telehealth, or in-person visits to identify needs for additional care or services.
