Position Type: Full-Time Schedule: Monday - Friday, 8am-5pm with potential on-call hours Salary Range: $100,000 - $107,000 per year Sign-On Bonus: $10,000 available! Qualifications: • Associate's degree in nursing; BSN preferred. • Active registered nurse license in the state of practice. • Current CPR certification (required in Georgia). • A minimum of one year of nursing experience is preferred; one year is required in California. • Experience in hospice, home health, medical-surgical, or specific populations (e.g., geriatrics, pediatrics). Responsibilities: • Conduct thorough nursing assessments and formulate personalized care plans in collaboration with the interdisciplinary team, addressing all dimensions of patient needs. • Ensure accurate and timely documentation of patient care and updates, adhering to internal protocols and regulatory standards. • Coordinate care activities to maintain continuity and quality, including scheduling visits, managing medications, and collaborating with physicians. • Educate and support patients and caregivers while effectively responding to urgent needs during on-call assignments. • Supervise LPNs, LVNs, and hospice aides, engaging in interdisciplinary meetings and ongoing education. • Evaluate patient needs via phone, telehealth, or in-person visits to identify any additional care requirements.
