Position Overview: As a Hospice Registered Nurse (RN), you will provide outstanding end-of-life care to patients and their families. You will bring both medical expertise and emotional support to your patients, playing a vital role in an interdisciplinary team focused on enriching the quality of life during this significant phase. Key Responsibilities: • Deliver compassionate, home-based care to hospice patients with a prognosis of six months or less to live. • Collaborate with a dedicated interdisciplinary team (physician, social worker, chaplain, CNA, HHA, and volunteers) to create and implement personalized care plans. • Conduct comprehensive patient assessments, monitor vital signs, evaluate respiratory function, and manage pain via prescribed treatments and medications. • Ensure the availability of essential medical equipment, oxygen, and medications for patient needs. • Educate patients and their families on care plans, medications, hospice objectives, and expectations. • Provide holistic care addressing the physical, emotional, and spiritual needs of both patients and their families. Qualifications: • Active, unrestricted RN license in the State of Michigan. • Minimum of one (1) year of nursing experience; hospice or home health experience is preferred. • Background in community health or Medical-Surgical settings is a plus. • Reliable transportation, a valid driver’s license, and current car insurance are required.
