Our clinicians work together to ensure symptom control, pain management, and continuity of care, guided by a collaborative plan and a commitment to excellence.
Schedule: Monday - Friday with on-call rotation (back-up)
Territory: Mobile county communities

WhatYoullDo:
• Deliver personalized hospice carethat honors each patients unique needsdocument every step to ensure accuracy and continuity.
• Lead with empathy during home visits, assessing patient and family needs and crafting care strategies that prioritize comfort, dignity, and peace.
• Empower families and caregiversthrough clear education on hospice services, symptom management, and available support resources.
• Guide and mentor your team, training Hospice Aides and LPNs to deliver safe, effective, and compassionate care at home.
• Collaborate seamlesslywith physicians, interdisciplinary teams, and facility staff to create smooth transitions and coordinated care plans.
• Provide hands-onexpertisein pain control, medication administration, and symptom reliefbringing comfort when it matters most.
• Be a trusted resource after hours,participatingin on-call rotations to ensure patients and families receivetimelysupport on weekends and holidays.
• Maintain compliance and eligibilityby monitoring patient status and completing all certification and recertification documentation with precision.

• Active RN license in state of residence
• Current CPR certification
• Valid drivers license, current carinsuranceand reliable transportation
