Position: Hospice Registered Nurse (RN)
Position Type: Full-Time
Remote/Virtual Position: No
Coverage Area: Branford and surrounding areas

Schedule: Monday - Friday 8am - 5pm plus 1 weekend day back up on call every 6 weeks
$10,000 Sign on Bonus

Be the Best Hospice RN You Can Be
If you meet these qualifications, we invite you to apply:
• Associates degree preferred
• Registered Nurse license required in state(s) practicing
• Current and valid CPR certification preferred - required in Georgia
• Minimum one year of experience preferred - required in California
• Experience in hospice, home health, general medical-surgical or specific patient populations (e.g., geriatrics, pediatrics)
Responsibilities:
• Conduct nursing assessments and develop individualized care plans with the interdisciplinary team (IDG), addressing patients' physical, emotional, social, and spiritual needs.
• Maintain accurate documentation of nursing care and patient status to comply with protocols and regulations.
• Coordinate patient care activities such as scheduling, medication management, and interdisciplinary communication.
• Educate patients and caregivers while addressing urgent needs during on-call or admissions assignments.
• Supervise LPNs, LVNs, and hospice aides and actively participate in IDG meetings and ongoing clinical education.
• Support pre-hospice services by evaluating patient needs through various channels.
