Position: Hospice Registered Nurse (RN)

Position Type: Full-Time

Remote/Virtual Position: No

Coverage Area: Branford and surrounding areas

Salary: $90,000 - $105,000 / year

Schedule: Monday - Friday 8am - 5pm, plus 1 weekend day back up on call every 6 weeks

$10,000 Sign-On Bonus

Qualifications:
• Associate's degree preferred
• Current Registered Nurse license in your practicing state
• Valid CPR certification preferred
• Minimum one year of nursing experience preferred
• Experience in hospice, home health, or related fields is a plus

Key Responsibilities:
• Perform comprehensive nursing assessments and develop personalized care plans with the interdisciplinary team.
• Ensure timely documentation of care and patient updates following regulatory standards.
• Coordinate patient care activities, including scheduling, medication management, and communication.
• Educate and support patients and caregivers in managing their care.
• Supervise LPNs, LVNs, and aide staff as needed during care delivery.
• Engage in pre-hospice evaluations to identify additional care needs.
