What You’ll Do
• Provide one-on-one hospice nursing care to patients with life-limiting illness
• Serve as the primary RN Case Manager, guiding patients and families through the hospice journey
• Deliver pain and symptom management, medication oversight, and skilled assessments
• Care for patients across home care and long term care settings.
• Support families through end-of-life decision-making with empathy and clarity
• Collaborate closely with physicians, social workers, chaplains, and hospice aides
• Complete accurate, compliant documentation while focusing on patient-centered care
