Your Responsibilities: • Manage a primary caseload of patients and families, overseeing all stages of the nursing process, including assessment, planning, implementation, and evaluation. • Gather and assess vital information regarding patients’ physical, psychological, social, and spiritual needs to facilitate interventions across the interdisciplinary team. • Communicate effectively with attending physicians, the interdisciplinary team, and other agencies to ensure well-coordinated care. • Conduct regular patient visits to reassess needs and update care plans. • Perform clinical pronouncements of death and provide compassionate family support. • Ensure accurate documentation of patient records, charts, and reports while preparing care plans promptly. • Supervise the delivery of care by Home Health Aides in accordance with established guidelines. Qualifications: • Your background should include graduation from an accredited nursing program. • You must be a licensed registered nurse in your practicing state. • Current CPR and First Aid certifications are required. • Knowledge of quality assessment and assurance procedures will be beneficial. • A valid driver’s license and proof of insurance are mandatory.
