
Hospice RN Case Manager in Bessemer, AL
Job Description

In this role, you will:
• Assume full nursing responsibility for delivering the Plan of Care for each hospice patient.
• Conduct the initial nursing evaluation to determine eligibility for hospice services within forty-eight (48) hours of referral.
• Identify the physical, psychosocial, and environmental needs of the patient and family, reassessing them no less than every fifteen (15) days.
• Initiate and coordinate the plan of care, documenting problems, goals, interventions, and responses from patients and families regarding hospice care.
• Collaborate daily with patients, families, attending physicians, and other interdisciplinary team members to provide comprehensive care.
• Instruct and supervise patients and families in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient's condition.
Experience Requirements:
Minimum one year of clinical experience.
License Requirements:
• Current RN licensure in the state of practice.
• Minimum one year of clinical experience.
• Current CPR Certification.
• Valid driver's license, vehicle insurance, and access to reliable transportation.
