Responsibilities
As a Hospice Transition Navigator, you will:
• Represent Enhabit in transitional care activities and engage with patients, families, physicians, hospitals, senior living communities, and other health institutions to inform them about our services.
• Integrate clinical guidelines and protocols to establish patient-centered transition plans that enhance quality and efficiency of care.
• Act as a public awareness advocate and take charge of educating stakeholders about the hospice services we provide.
Qualifications
Essential:
• Graduate of an approved nursing school.
• Current state nursing license.
• Minimum of one year home care experience.
• Understanding of federal, state, and local regulations governing hospice operations.
• Basic technology skills, including proficiency with mobile devices.
Preferred:
• Experience and knowledge in the field of hospice are preferred.
Requirements
• Valid state driver license.
• Automobile liability insurance as required by law.
• Reliable transportation in good working order.
• Able to safely drive in various weather conditions.
