THIS ROLE IS HYBRID WITH 75% TRAVEL. MUST RESIDE IN ONE OF THE LISTED COUNTIES FOR CONSIDERATION. • Monday through Friday • 8:00 am - 5:00 pm EST (Some flexibility in the schedule) • $37.75 per hour • Must reside in Orange-Osceola-Brevard-Seminole Counties Position Summary The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.
