Job Description
A company is looking for an RN - Patient Clinical Services.
Key Responsibilities
Assist patients via phone and electronic platforms to determine care needs and provide health information
Evaluate and document patients' concerns while coordinating with Primary Care Center staff
Manage incoming patient care requests and triage concerns based on urgency, directing patients to appropriate resources
Required Qualifications
Licensed Registered Professional Nurse (RN) in Illinois
Licensed Registered Professional Nurse (RN) in Indiana within 1 year
College Diploma in Nursing
Experience in patient care coordination or triage
Ability to support and mentor new nursing personnel after probationary period
