In Home Care (RN) in Virginia Beach, VA

We are seeking an experienced Registered Nurse (RN) to provide skilled home health services on a PRN basis. This role includes Start of Care (SOC), Resumption of Care (ROC), routine skilled visits, and patient education in accordance with physician orders, Medicare Conditions of Participation, and agency policies. Key Responsibilities • Complete Start of Care (SOC), re-certifications, and routine skilled nursing visits • Perform comprehensive patient assessments and develop individualized plans of care • Accurately complete OASIS documentation within required time frames • Administer medications, wound care, disease management, and patient education • Coordinate care with physicians, therapists, and interdisciplinary team members • Maintain compliance with Medicare, state regulations, and agency standards • Complete documentation timely and accurately in the EMR Qualifications • Active Registered Nurse (RN) license in the state of practice • Minimum 1 year of skilled home health experience preferred • Experience with OASIS documentation strongly preferred • Current CPR/BLS certification • Valid driver’s license, reliable transportation, and auto insurance • Strong clinical judgment, communication, and time-management skills What We Offer • Competitive per-visit pay • Flexible scheduling • Supportive leadership and clinical resources • Opportunity for consistent PRN visits as census grows Schedule • PRN / As needed • Weekday and/or weekend availability preferred
