Schedule: Monday to Friday, 8:30 AM - 5:30 PM

Primary Responsibilities:
• Deliver individual and group patient care.
• Collaborate with primary care providers to educate and monitor patients.
• Identify patients who need educational support and follow up on their progress.
• Manage a caseload of patients independently with thorough documentation in electronic medical records.
• Stay updated on the latest developments in patient care.
• Provide education to nursing staff as required.
• Utilize developed curricula to support patient care.
• Ensure professional and courteous interaction with all patients.
• Facilitate self-management education during both group and individual sessions.
• Perform additional duties as assigned or necessary.

Required Qualifications:
• Graduation from an accredited School of Nursing.
• Active RN license in the state of New York.

Preferred Qualifications:
• Experience with Epic electronic medical records.
• Background in an ambulatory clinic setting.
• Proficiency in software applications including Microsoft Excel, Word, and Outlook.
• Exceptional organizational, interpersonal, and communication skills.

Compensation for this role ranges from $28.27 to $50.48 per hour based on various factors, including local labor markets, education, and experience. We also offer a comprehensive benefits package, incentive and recognition programs, stock purchase options, and 401k contributions, all subject to eligibility requirements.
