Intake RN in Brooklyn, NY

We are seeking an experienced Intake Registered Nurse (RN) to join our growing home care team. This role is critical to ensuring timely, accurate intake and coordination of services for patients entering home care. The Intake RN serves as a clinical liaison between referral sources, patients, and internal teams to ensure compliance, quality, and continuity of care. Key Responsibilities • Review and process new patient referrals for home care services • Conduct clinical intake assessments and determine eligibility for services • Verify orders, diagnoses, and required documentation for compliance • Coordinate with physicians, hospitals, discharge planners, and referral sources • Communicate with patients and families regarding services and next steps • Collaborate with scheduling, clinical, and administrative teams • Ensure compliance with NYS DOH, CHHA, and agency policies • Maintain accurate documentation within the EMR system Qualifications • Active New York State Registered Nurse (RN) license (required) • Minimum 1–2 years of home care, intake, or clinical coordination experience preferred • Strong knowledge of home care regulations and documentation requirements • Excellent communication, organizational, and multitasking skills • Proficiency with EMR systems and Microsoft Office • Ability to work efficiently in a fast-paced office environment
